Welcome to the new Health Self Assessment page. This page may grow in the next year as teams across the North West develop their own information and documentation regarding this process. This information can be forwarded for inclusion to Sue Smith – address details are below.
In the meantime, though, the aim is to make it easy for you to get the papers you need to do your annual self assessment in the coming year. This information has been taken from the Valuing People Website and includes the accessible information developed by Inclusion North as part of the Health self Assessment Process.
Following the information about the Health Self Assessment process a copy of the agreed timelines is attached for your information. Next to help you understand the governance structure followed for this process a copy of the reporting mechanisms is included. All information gathered and the progress to date will be reported via the bi monthly Health Equality Group. This Group reports directly into the Regional Valuing People Board.

A revised self assessment Framework for 2010
People who had been working with the Framework in different parts of the country met to review the Framework and linked papers in November 09. You will need to read the revised 2010 Framework carefully to be aware of the changes. The Guidance Notes will also tell you more about the changes made.
There is an additional (sixth) appendix this year about the Annual Health Check. The Guidance Note will also tell you more about the appendices

General Feedback Form
This form allows you to record the level of progress you feel you are at after your self assessment exercise. It will also allow you to record the really goof things you are doing, as well as your priorities for the year ahead. Once completed, your PCT returns it to the SHA.

This additional form has been included this year at people's request, in order to pull out more clearly the data/measures requested - and also to be more specific about the suggested evidence or supporting information people should provide. Again, it should be completed as indicated, and your PCT returns it to the SHA.
