Welcome to the 2011 Health Self Assessment page.

As you are aware the North West is now on year two of the Health Self Assessment. The results of year one can be seen on the learning disability Public Health Observatory via this link. http://www.improvinghealthandlives.org.uk/self_assessment/regions/index.php?region=Q31&map

The regional report , which includes evidence of good practice that is happening acrooss the North West is available below.

This year the paperwork for the Health Self Assessment has been updated and made clearer for people to understand. The paper work is below.

All areas have been sent this information and they are busy arranging their big health days. This years Health Self Assessment will be submitted by 23rd September and the valiadtion meetings are booked for November.

Launch of New & Much Anticipated CHANGE General Picture Bank

The new CHANGE General Picture Bank will contain a comprehensive collection of over 1000 updated pictures. The range of topics has been extensively expanded to include:

·         Independent living, transport, money, community and support

·         Education, employment, planning, time and meetings

·         Rights, government, law, justice, violence and abuse

·         People, diversity, family, friends, emotions, sex and relationships

·         Health, wellbeing, leisure, exercise, healthy eating and medical care

The new CHANGE General Picture Bank includes a guide which explains how to produce easy read documents, and document templates covering personal budgets, meeting agendas and many other subjects.

A revised self assessment Framework for 2010

People who had been working with the Framework in different parts of the country met to review the Framework and linked papers in November 09. You will need to read the revised 2010 Framework carefully to be aware of the changes. The Guidance Notes will also tell you more about the changes made.

General Feedback Form

This form allows you to record the level of progress you feel you are at after your self assessment exercise. It will also allow you to record the really goof things you are doing, as well as your priorities for the year ahead. Once completed, your PCT returns it to the SHA.
