Older People . . .
NWTDT/ Pathways have always worked to support Older Citizens and their families. In recent years however our experience has informed the extension of our formal programme to incorporate more work to support older citizens, their families and support services (statutory and provider).
Older People who have care and support needs should be able to exercise choice about how and where their needs are met. It seems that many people we have spoken to echo the same frustrations with regard to a lack of consultation with older people and are often in receipt of poor support which does not best meet their needs at a time when it is crucially needed.
For many years NWTDT/ Pathways have been working to increase social inclusion and develop leadership to enable older citizens, their families and the people who support them make informed decisions in order to have the best life possible.
We have been working in close consultation with older people who are currently receiving support. The aim of the work was to learn about the things that matter and are valued by older people and how this can be used to inform a better response which can be provided to meet the changing needs of the older person and their families.
Older citizens and their families are welcomed as individual members of Pathways Associates.
 

In order to achieve this aim we run many training courses for individuals, families and professionals. Annual events bring people together to hear how they can influence services and policy at national, regional and local levels.
The NWTDT has been working in close consultation with older people who are currently receiving support. Building on the vast knowledge end experience of the NWTDT in relation to person centred approaches and the benefits of this to people who use services, families and services a framework has been developed that supports a person centred approach for older people. The framework looks at how services and communities can, and are working together with older people and their families through a wide range of positive experiences both within the community and through the services they may choose. An easy to use handbook has been developed to support this.
The handbook developed for older people by older people is now proving to be a better way of working out better support for those who are particularly vulnerable and in need of continuing care.
