Person centred approaches to transitions for young people. . .

  1. Create the purpose for how we will work with young people.
  2. Describe our roles and responsibilities to those young people who are using services. Consult families in our commitment to work with each other.  This will outline how we will work and what to expect, and any issues to resolve.
  3. Creative leadership, Identify those young people who would like to plan. Identify the peer mentors who will be planning guides.

 

Model of practice, Live planning.
 Advantages of this process

  • People support each other and Share ideas about what is possible (and may support each other informally after the event)
  • People offering support (e.g. Support planners, care managers and circles of support) work in a supportive environment, with a trainer to assist them as necessary. It therefore builds confidence and skills
  • A days training for support planners in support planning and developing creative options
  • Support planners then support someone over 2 – 3 days as part of ‘Planning Live`
  • Support planners are trained to deliver ‘planning live’ themselves by having a day with the trainer to prepare, and then to co-lead one ‘planning live’ event with the trainer, and then to run a second themselves with the trainer.

 

Each one teach one.


It is important that support planners receive support and coaching themselves. This could be a day every month or 6 weeks initially, for coaching and problem solving. Support planners will also be the people who see very quickly what the challenges and difficulties are in delivering individual plans, and there needs to be a way for their learning to be shared with the project leads/groups who are managing the local development of self directed support.

Planning Live! offers a way to develop plans that have young people in-control support plans, build local informal networks and develop support planning capacity. There are opportunities to connect this to the experience and information on peer mentoring, and to develop local social firms of young people and their allies.

 

Person centred thinking and approaches.

Working with practioners to support plans with young people resulting in them not having just a plan, but a better life. This 2-day course will provide real practical tools to ensure people’s plans are in forming our duty to support young people to have a life that is important to them and address issues of support, health and well-being. Some of the tools and skills will include, facilitation of person centred planning, Action planning, person centred reviews, person centred health care plans.

 

Action Learning sets. Encouraging a spirit of inquiry.

Creating space every 6 weeks for dialogue interaction and critical engagement, for those who are providing support to planning with young people.
Each action learning set meets to explore areas of mutual interest to learn from each other, and work across boundaries in supporting young people going through transition.
